FAQs about Processing of Request for Correction of Entry of Grade and Completion of Incomplete Grade

 Types of Completion Form

Students shall be graded or marked in accordance with the university system. Any student who is studying under the PUPT institution that has justifiable reason is suitable in this registrar services' completion form.

Late Reporting

Students who haven't yet received any remarks from their professor are qualified in this type of completion request.

Completion of Incomplete Grade

Students who have received a temporary “Incomplete” remark from their professor due to acceptable reasons and have been given the chance to comply with such requirements, are allowed to apply in this completion request.

Correction of Entry

Students who have received a mistaken calculated entry of grade or remark has their right and is valid in this field of completion request.

Steps on Processing a Completion Form:

  • Pay at the PUPT Cashier Office
            - The completion form application requires a payment of ₱30.00 only.
  • Show your receipt at the registrar office and have them validate it.
  • Scan the receipt for you will have it uploaded on the Registrar Services System.
  • Create an account here at the RSS. Fill out all the areas asked by the system. Get the verification code through your given email then finally login.

Upon having your account logged in:

  • Fill out the Completion Form, we remind you to only put honest data for this application between student-professor connections.
  • Do not forget to submit the scanned copy of the receipt you got from the cashier that has the validation of the registrar.
  • After submitting all the requirements, the process continues. Please wait until it is verified and authenticated.

 Who are qualified for requesting?

  • Students who have “INC” mark. It is temporarily given to a student who may pass the subject, but not yet complied with all its requirements.
  • Students that have concern with their grades; note that the professor must be aware of the student’s complaint and is in need to request for the change of his/her grade.

 Where will I get the receipt?

Upon the request, the system will generate a printable/downloadable stub that you will present to the cashier department on PUP-T. You will pay an amount of P30.00.

 What are the requirements needed in requesting for change of grades and completion of incomplete grades?

Submit or upload the filled out completion form and receipt.

 What will I do after submitting the receipt and the filled out form?

Please wait for your documents to be processed. If your documents are already verified, you will received a printable copy of your new grades.

 When should I request for correction of entry of grade and completion of incomplete grade?

As soon as the grades depict on the sis. But please be reminded that you must consult first your professor regarding on your concern to your grades before filing a request. 
 Correction of entry should be done within one semester upon receipt of the grade. The student who passes the subject, while it is still not compliant with all the requirements, is temporarily granted Incomplete (Inc.) Processing such requirements shall be fulfilled within a period of 1 year from the end of the term; otherwise "No Credit 👎 or a failing mark" shall be given.
